We are looking for a Member Care Coordinator to enhance and manage processes that support Brightside patients’ treatment and plan of care. This individual is responsible for and dedicated to the clinical and administrative coordination of Brightside patients. These members may have complex behavioral and mental health care needs and need support and connections to appropriate levels of care. This individual will work closely with clinical and administrative teams at Brightside to align care transition activities and achieve desired goals and metrics

What you’ll be doing as Member Care Coordinator:

  • Work collaboratively across departments to facilitate enrollment and discharges into and out of Brightside

  • Manage prospective and current member clinical care transitions within Brightside (e.g. enrollment, care navigation, care coordination, and discharge planning for mental health services)

  • Track members throughout the care journey and help facilitate various aspects of care coordination based on members’ needs

  • Develop and oversee relationships and referral processes between Brightside and external healthcare partners in collaboration with Brightside’s Commercial team

  • Build interdepartmental relationships to assure collaboration and efficient operations both within Brightside as well as outside.

  • Work collaboratively with all care team members to develop and implement treatment plans that support a member’s plan of care and address their unique needs and preferences.

  • Communicate regularly with the clinical team to support member referrals to outside treatment settings.

  • Ensure timeliness of services as well as appropriate utilization of services, optimizing service delivery and compliance

  • Provide advocacy & education for Brightside members to ensure high-quality care coordination services

  • Support team in developing and implementing virtual care coordination interventions and member engagement strategies

  • Ensure compliance with all applicable laws, regulations, and company policies related to care coordination

Requirements:

  • Have a Master's degree in work related discipline/field from an accredited college or university

  • Unrestricted state licensure preferred for either LCSW (Licensed Clinical Social Worker) or LPC (Licensed Professional Counselor)

  • Minimum 2-3 years post-licensure experience

  • Has experience in care coordination or case management, preferably in a mental health or behavioral health setting

  • Resides in PA, NY, MA or VT
